What waiver programs do you accept?

We serve individuals authorized through BI (Brain Injury), CAC (Community Alternatives for Disabled Individuals), CADI (Community Access for Disability Inclusion), DD (Developmental Disabilities), Elderly Waiver, Alternative Care, and State Plan programs.

You can contact us directly or have your case manager make a referral. We respond within 24 hours, conduct an assessment, and work to start services as soon as authorization is in place.

We provide services across all of Minnesota. Whether you are in the Twin Cities metro area or greater Minnesota, our team is ready to support you.

Most waiver-funded services require authorization through a case manager or lead agency. If you do not yet have a case manager, we can help connect you with the right resources to get started.

Individualized Home Supports (IHS) focuses on habilitative skill-building — training individuals to develop daily living skills. Individual Community Living Supports (ICLS) provides bundled assistance for daily living, similar to assisted-living-level support, delivered in your own home.

Yes. All staff undergo background checks, receive comprehensive training in person-centered care, and are matched to clients based on needs, preferences, and compatibility.
